Central Valley Community Partners LLC is a CMS Direct Contracting Entity.

CVCP is pleased to be named a Centers for Medicare & Medicaid Services (CMS) Direct Contracting Entity (ACO REACH). This is a special program for Medicare beneficiaries who have Original Medicare. Beneficiaries are either aligned by CMS or can voluntarily opt into the program. Beneficiares in the ACO REACH program receive extra benefits while retaining the freedom of choice
to go to any doctor, hospital or other healthcare provider that accepts Medicare.

ACO REACH is a group of doctors, hospitals, and other healthcare providers who agree to work together to keep Medicare patients healthy. CVCP providers agree to work together to see that beneficiaries get the right care at the right time. CVCP helps providers work together to give beneficiaries better care. CVCP coordinates patient care according to individual medical needs and treatment choices. CVCP protects beneficiary medical records and privacy and works to reduce duplicate tests and duplicate paperwork that cost beneficiaries time and money.

CVCP will offer ACO REACH beneficiaries enhanced engagement and coordination of care through incentive programs and cost-sharing. CVCP partner with beneficiaries to provide necessary preventive care, including adherence to treatment plans and prescriptions, follow-up care, and management of chronic conditions.
